I take a friend to get shots here and they control her money and pay her bills. They give here money weekly but whoever is passing out money is stealing the money. I also have talked to other people who has not recieved their money. Not a good place for people who are in need of help.

If I cold give 0 stars, I absolutely would! I cannot continue with this company due to its lack of professionalism. No one answers the phone, and when someone does, they don't even know what to do. I have been trying to get any appointment for the longest and keep getting the runaround. If you are not interested in helping your patients, please just close this business. Highly unprofessional and I would never recommend anyone here.

Just spent 7 days there,it was Hell. First off I was just trying to get some help for health anxiety due to medical stuff I been going threw last couple years. It litterly felt like prison. The beds suck forget about getting sleep . One flat ass pillow. The food always cold . The coffee sucked and only juice and water. I drink caffeine so yeah I went threw the caffeine withdrawal so bad. I was so sick by the time I left there. I had a stroke over a year ago and take a baby asprin every day and potassium. I COULDNT get my meds for 4 days and had to fight with rude nurses to get my inhaler because they had none. 3 people there that had asthma and we all had to fight with the nurses, they finally brought mine up from down stairs. They should also have a nebulizer there for breathing treatments if need be. Had to fight with the nurses for 2 days to see the doctor and when I did he just wrote some stuff down didnt do nothing for me. The physio doctor hardly ever looked at me when I seen him he stares at his laptop the whole time and he kept asking me about my sister. I have no sister so dont know if the hospital I came from had me confused with someone else or what, which might explain how I ended up there in the first place. 2 of the techs there were great wish I could remember their names. Alot of fights and craziness going on there. I felt worse coming out than I did going in for sure. It has taken me 3 days to start feeling normal again.

Absolutely the lowest level of care one can get is at vienna rural health...I have two years worth of complaints but my mom is one of these people who just keeps taking and taking others Disrespect...BUT she is FINALLY done now!..My mom described this last visit as "TRAUMATIC!!" She kept saying it felt like it wasn't real.."all day i kept playing in my head what they did to me and asking myself , what did I do wrong!!!" She is 76..happiest person ever and today , this facility broke her spirit!! .she was repeatedly disregarded regarding my dad's Healthcare..A nurse literally told my mom to not raise her voice at her..my mom never raises her voice, my mom was addressing a concern for my father and was Being belittled by the people we trusted our fathers care too..they are horrific people!! Been a repeated pattern of dismissal, condensing explanations, reckless neglect under gauze of self appointed entitlement to themselves, which has been held over her and my Dad's head for YEARS!! My mom's a nurse, been married to my dad for 57 yrs, his primary caregiver BUT they make her feel 2 inches tall after most of their past visits...dismissing her voice and concerns....this last one was the breaking point!! This is truly the LAST STOP for any form of Healthcare!

Went there after having some dental work done hoping to get some antibiotics I knew I had an infection Jessica Oslund came in claiming to be my provider. She was rude didn’t even look at my mouth and proceeded to tell me she didn’t see an infection. Now I’m not a doctor myself but an infection does not always show signs. And proceeded by she’s been getting a lot of dental work done too so it’s normal. I asked to speak with another provider she claimed I couldn’t. I was on the phone with another clinic and she proceeded to get loud and tell me she didn’t have time for me to be on the phone and that I need to leave right now. I will never go back to that clinic and she’s the most unprofessional provider I’ve ever come in contact with!

I took my son to Crusader Community Health in Rockford for a routine sick visit, and the experience was extremely disappointing and concerning. The provider made unfounded assumptions about my child’s health and reported false concerns to DCFS, despite my son’s complete medical clearance from multiple reputable hospitals, including DuPage Pediatrics and Northwestern Medicine. Rather than reviewing his full medical history or communicating with his established doctors, Crusader jumped to conclusions that caused unnecessary stress, wasted time, and triggered a DCFS investigation—one that has since been fully cleared by verified medical evidence. During the visit, they used an incorrect blood pressure cuff, leading to claims of low blood pressure without any proper follow-up instructions. Additionally, baseless accusations of malnutrition were made, despite the fact that no care plan had ever suggested this concern. These errors reflect a serious lack of proper equipment, training, and professional judgment, putting children at unnecessary risk. Parents deserve clinics that listen, review records thoroughly, and communicate responsibly before making damaging claims. I will never return to Crusader and strongly advise families to seek care from providers who practice with accuracy, professionalism, and compassion. This is a public warning: avoid this location. The employees and practices here fail to meet the basic standards of medical care, and the system is failing the families they serve. Karma—what is above is as below. I stand by my statement.

This place is only about taking your money. They do not care if they mess up your paperwork and they will not give a refund especially when you brought in the state papers for them to fill out and they lied to you and tell you that they know what they’re doing, but their counselor doesn’t know what they’re doing at all. They even have residences that are living there for rehab treatment doing their paperwork and answering phones what a way to save him money because they are paying him like wow but this man that say he owns this says that he will not give me my money back he’s only about taking people’s money and he doesn’t give a crap about you and makes you think that you are this person that you’re not so would not recommend going there at all and the counselor doesn’t even speak our language and the whole place is really sketchy and the man also tried to tell me that I went somewhere else and they gave me what I wanted to hear and I didn’t want to accept the truth from them when they brainwash you they try to like really this man got jokes because I even called a made a police report on them you messed with the wrong one because if i was such as a person as they were trying to say then, how am I a CNA and how do I maintain a life and I never miss work and I put a roof over my family‘s head he also didn’t like that comment and hung up the phone on me

My turn to review… I get to do two as I’ve actually been through this program 3 times… one successfully 5 years ago and was way better. The second time 4 months ago I had to stop the program early due to audiology appointments and dealing with vertigo. Which means a few times I had to step out to make appointments calls or respond to my chart. Which was in advance, made aware that I was struggling with hearing issues and ringing and dizziness that was spiraling my panic attacks. They held all of that against me when I came back 4 months later for a fresh start and threatened to discharge me the first day for any future phone use.How encouraging. I also had one food delivery that went to the wrong desk… and this is the welcoming chat I got my first hour there in the program. Let’s not forget Bonnie starting me on EMDR and not keeping consistent appointments then leaving the practice.

The absolute worst. The will deliberately lie under oath and yet call the parent deceitful. Very sad how they keep families apart for a paycheck and not follow their own policies. They “have concerns” but will not tell you what they are. They will lie about your records even though their name is tied to the documents. Sad identical service plan from Family Core case in Peoria county was used in another state and I got my child back out of state, but caseworker used manipulation and lies to get my rights terminated to my children. They refused to get DNA test for my children’s father after he asked repeatedly for it so he could do services and see his girls. They all need psychological evaluations themselves as the caseworker I have liked to talk about she wished her baby daddy was locked up for child support. Do your job and help people not destroy what they are struggling to get back. I would never recommend this place to my worst enemy. They will lie on other caseworkers from different agencies with no actual proof of what is being said to be true. If you can prove they are lying in court seriously talk to the state’s attorney’s office and go after them for contempt. All they is criminals and take parents to court at a civil level. Fight for criminal charges and prosecution if you can prove your caseworker has lied. You fight for your kids, fight for the injustice of the criminal activity this agency does to your families.

My daughter has been a patient at this office for years, and I was seen here throughout my pregnancy as well, so I came in to enroll my son. I was checked in and had already started filling out paperwork before the front desk staff guy told me they “don’t accept my insurance.” This makes absolutely no sense, considering my daughter was just seen there a couple days ago and my insurance has not changed at all. Instead of double checking or trying to resolve it, the front desk employee simply turned us away after we had already gone through the check-in process. It was a complete waste of time and incredibly frustrating, especially having to bring my child in just to be turned away like that. This should have been verified before check-in, especially for a family that has been established with this office for years. The way it was handled was rude and lazy. No effort to help, no explanation, nothing. If this is how they treat people who’ve been coming here for years.

Dentistry- 0 stars PCP- 5 Stars Do not waste your time coming here for any dental work. I have come now for two separate appointments, and I have still not been seen for a routine cleaning. Both times I have arrived early to my appointment and waited ~45 minutes to be seen, and yet I am still asked to schedule a 3rd appointment to get a cleaning, which is not available for another 3 months! I've had x-rays and cleanings done at other places all in a single 1-hour visit, so I do not understand how they are running things here. I will not be returning. PCP was great on the other hand. Quick service, answered all my questions, and great bedside manner. The only thing I recommend here.

Healthcare Alternative Systems really seemed to say, "Let's make mental health even more stressful." After waiting three months for my appointment, I missed it because the receptionist - who was completely unprofessional - gave me the wrong date over the phone. I thought I had everything figured out, but I was wrong. Not a single apology was offered. Calling them is pointless; their phones might as well be decorations. No confirmations, no reminders - just confusion and bad vibes. If you're struggling, try journaling or screaming into the void - both are more reliable and helpful than this place could ever be.

So, I take it the same woman who answers the phone for the, now shutdown men's residential location, has been working there for about 5 years? Has no one ever told her she needs a customer service class? She's very rude. I'm aware of HIPAA laws..... and customer service. I get it isn't a customer service position, per say, but you still need proper manners when answering phones and de-escalation skills. This woman will just read off I cannot confirm or deny that individual blah blah blah..as loud as she can and over the callers voice and words etc. I am also listed in a resident's file as having a full release. Other people have been civil and nice at times, but this woman has no people skills whatsoever.

I called ahead specifically for adult autism testing and diagnosis and was told this is a service they provide. Based on that, I scheduled and spent about $100 on Lyft rides to get there. I was there for nearly four hours, and it became clear no one understood I was there for a diagnostic evaluation. The assessment felt extremely generic and surface-level, with no explanation of what was being done or why. They then sent in an occupational therapist, which made no sense for a diagnostic appointment. He gave no direction, and I had to repeatedly ask what he needed from me. There was no clarity about the purpose of that time at all. Afterward, I was told I would see a nurse practitioner next, who may or may not do autism testing. No clear plan, no explanation of the diagnostic process, and no indication of who would actually diagnose. I was also told they would call me to schedule once the nurse practitioner returned from being sick. That was two weeks ago, and I’ve heard nothing. The entire experience felt disorganized, poorly communicated, and unprofessional. I came in for a specific service, was told they provide it, and instead got a vague intake process with no clear path forward. This was a complete waste of time and money.

The Pavilion was absolutely amazing and so helpful to my organization. They constantly kept me in the loop of what was going on and what was needed. I spoke with Jasmine in Intake/Admissions throughout the day and she was absolutely amazing. She was extremely helpful in everything that she did for my client. Jasmine is such an AMAZING asset to the Pavilion Foundation. I've worked in the medical field for 17 years and I've never had someone who was so helpful and so caring as much as Jasmine was. I want to personally thank her. Honestly thank you for making this process that seems to usually be so difficult and turned it around and made it so easy to find placement. Thank you again!

Frequently Asked Questions

How should I compare mental health providers in Illinois?
When comparing providers in Illinois, consider location relative to where you live or work, the specific services offered, levels of care available (such as outpatient, intensive outpatient, or inpatient), therapy approaches used, conditions treated, hours of operation, whether a website is available, accreditation status, and reviews when listed. Not all details are available for every provider, so it can help to check each facility listing directly.
What should I ask before contacting a provider in Illinois?
Before reaching out to a provider, it helps to know what you are looking for. Consider asking about current appointment availability, accepted insurance plans and self-pay or sliding-scale options, whether telehealth appointments are offered, which specific services are currently active, and whether the provider has experience treating your particular concern. Availability and services can change, so confirming details directly with the facility is recommended.
Are all services listed on Mental Health US available at every facility?
No. Services vary by facility and may change over time. The information shown in each listing reflects what was available when the record was created or last updated. Always confirm current service availability, hours, and intake procedures directly with the provider before visiting or calling.
Can I find both mental health and substance use treatment providers in Illinois?
Yes. Mental Health US includes Illinois providers that offer mental health services, substance use treatment, or both. You can use the available filters to narrow results by treatment needs, or review individual facility pages for more detailed information about the services and conditions each provider treats.
What if I cannot find the right provider in my city?
If you do not find a suitable provider in a specific city, try broadening your search by browsing all Illinois providers using the statewide view, or use the filter dropdowns to search by condition treated, therapy type, or level of care without entering a specific city. You can also browse nearby city pages to see providers in surrounding communities.
