Project Transition

Project Transition, based in Philadelphia, PA, provides comprehensive psychiatric rehabilitation and behavioral health services for adults and young adults living with serious mental illness, co-occurring substance use disorders, and intellectual/developmental disabilities (I/DD). With a person-centered approach rooted in community integration, Project Transition empowers members to thrive through structured support and therapeutic care.

Since 1982, the organization has offered residential mental health services, pairing independent community living with daily access to outpatient and day programs. These programs include skill-building workshops, medication management, therapy sessions, and peer support, delivered by a multidisciplinary team of mental health, substance use, and I/DD professionals. Additionally, the Systems of Support (SOS) program provides crisis prevention and stabilization for individuals with dual diagnoses, ensuring a continuum of care designed to promote recovery, independence, and long-term wellness.
