Telecare – 12th Street House in Bend, Oregon, is a five-bed residential treatment program dedicated to supporting adults with serious mental illness who require 24-hour supervision and assistance with psychiatric recovery. Designed as a recovery-based residential environment, the program focuses on fostering independence, self-responsibility, and skill-building for individuals transitioning toward more independent living. The multidisciplinary team at 12th Street House includes psychiatric prescribers, qualified mental health professionals, recovery specialists, and peer support specialists who work collaboratively to create individualized care plans based on residents’ goals and aspirations.
