Sandra Eskenazi Outpatient Care Center

Sandra Eskenazi Mental Health Center’s Outpatient Care Center, part of Eskenazi Health in Indianapolis, Indiana, is a comprehensive community mental health hub. Located at 720 Eskenazi Avenue on the downtown campus, it offers hospital-based outpatient care for all ages—children, adults, and older adults—with an emphasis on accessibility, coordinated care, and sliding-fee options for uninsured or underinsured individuals.

This multidisciplinary outpatient setting supports those facing diverse mental health and addiction challenges through a continuum of services: individualized therapy, medication management, integrated dual-diagnosis treatment, case management, and specialized services like supported employment and residential options. It also houses the Dr. Alan & Diane Breier Prevention and Recovery Center (PARC) for early psychosis, serving individuals ages 14 to 35 with an OnTrack model of comprehensive coordinated care.

With a collaborative clinical team and evidence-based approaches—including CBT, DBT, TF-CBT, EMDR, MI and person-centered modalities—the center effectively treats conditions like schizophrenia, bipolar disorder, depression, anxiety, addiction and other mental health concerns.
