Pilsen Wellness Center - Outpatient Recovery Center

Pilsen Wellness Center – Cermak Location, situated at 2015 West Cermak Road in Chicago, Illinois, offers a wide range of behavioral health services for adults, families, and adolescents. This location is home to the Project for Assistance in Transition from Homelessness (PATH) Program, which provides outreach and referrals for individuals experiencing serious mental illness and co-occurring substance use disorders who are homeless or at risk of homelessness.
