PACE North

PACE North, located in Traverse City, Michigan, provides comprehensive, participant-centered care designed to support older adults in maintaining their health, independence, and quality of life while continuing to live in their community. Through a coordinated interdisciplinary team approach, PACE North delivers medical, wellness, and supportive services tailored to each participant’s unique needs. Care plans are developed collaboratively by physicians, nurses, therapists, social workers, and care coordinators, ensuring that medical care, therapy services, nutrition, and daily living support are seamlessly integrated.

Most services are delivered through the PACE North Adult Day Center, where participants receive medical care, therapy services, social engagement, and nutritious meals in a welcoming and supportive environment. When needed, additional services such as home health care, personal care assistance, transportation, and caregiver support are provided in the participant’s home. By coordinating all covered services under a single care model, PACE North helps reduce hospitalizations, supports aging in place, and provides peace of mind for both participants and their families.
