Care plans are developed collaboratively by physicians, nurses, therapists, social workers, and care coordinators, ensuring that medical care, therapy services, nutrition, and daily living support are seamlessly integrated.
Most services are delivered through the PACE North Adult Day Center, where participants receive medical care, therapy services, social engagement, and nutritious meals in a welcoming and supportive environment.
