Community Health Pavilion

Community Health Pavilion – Shadeland, located on the east side of Indianapolis, Indiana, provides comprehensive outpatient behavioral health and addiction services within a patient-centered medical home environment. The pavilion is designed to deliver whole-person care through coordinated services, offering support for both mental health and substance use challenges. Adults and children alike have access to a full spectrum of outpatient care, including partial hospitalization programs (PHP) and intensive outpatient programs (IOP), all designed to promote wellness and recovery while allowing individuals to remain in their daily environments.
