Pilgrim Psychiatric Center - ACT Team

Pilgrim Psychiatric Center – ACT Team, located at 116 Waverly Avenue in Patchogue, New York, is an outpatient behavioral health service focused on providing Assertive Community Treatment to adults, young adults, and seniors with complex psychiatric needs. As part of the Pilgrim Psychiatric Center network operated by the New York State Office of Mental Health, this ACT program offers intensive, community‑based support for individuals whose severe mental illness has not been adequately addressed through traditional services.
