Continuum – Behavioral Health & Recovery Services in Grants Pass, Oregon, is an outpatient behavioral health and substance use treatment center offering a full continuum of care for adolescents, teens, and adults. Formerly known as OnTrack Rogue Valley, the organization provides integrated mental health treatment and substance use disorder services designed to meet individuals at every stage of recovery. Services include outpatient care, intensive outpatient programming, partial hospitalization programming by referral, DUII and Minor in Possession education, peer support services, and comprehensive assessment and referral. Walk-in outpatient services are available, with evening and Saturday hours to increase accessibility.
