Outreach Community Health Centers

Outreach Community Health Centers, located at 220 West Capitol Drive in Milwaukee, Wisconsin, is a comprehensive Federally Qualified Health Center dedicated to addressing the full spectrum of health and behavioral health needs in the community. With over four decades of service, Outreach has evolved into a multi-site organization that delivers primary care, chronic disease management, dental care, OB/GYN services, pediatric and prenatal care, immunizations, lab testing, and pharmacy access. The Behavioral Health Services at Outreach offer compassionate, trauma-informed care for individuals and families, with a strong focus on promoting recovery and long-term mental wellness. Services include outpatient therapy, psychiatric medication management, specialized treatment for substance use and stimulant use disorders, and structured programs like Comprehensive Community Services (CCS) and the Community Support Program (CSP), which employ best-practice models such as Assertive Community Treatment. Their interdisciplinary team—consisting of psychiatrists, psychologists, nurse practitioners, case managers, peer supports, and care coordinators—ensures clients receive individualized attention in both individual and group settings.
